Imaging diagnosis of extrarenal Wilms tumor in a pediatric patient. Case report

Introduction: Wilms tumor is probably the most common renal malignant tumor in children. However, the extrarenal location of this type of tumor is extremely rare. Objective: To report the case of a pediatric patient diagnosed with extrarenal Wilms tumor of retroperitoneal location. Case presentation: We report the case of a 7 - years - old male patient who was brought for consultation due to pain in the left thigh and hip since one month ago. Physical examination showed positive Fabere-Patrick and Thomas maneuvers. A pelvic bone x-ray was performed, showing a mixed osteolytic and osteoblastic image affecting the iliac crest and left sacroiliac joint. The abdominal ultrasound showed a well-defined solid mass with heterogeneous pattern of approximately 6 centimeters in diameter in the left iliac fossa projection decided to perform a computed axial tomography of the abdomen and pelvis, where a mixed bone lesion was defined in the left iliac crest and bilobulated hyperdense mass occupying the left retroperitoneal space. Results of imaging studies showed an extrarenal retroperitoneal malignant tumor with invasion of adjacent bone and muscle structures; the tumor was biopsied and diagnosed as an extrarenal Wilms of retroperitoneal location. Conclusions:
The clinical radiological evaluation of this patient highlighted the importance of imaging studies to accurately diagnose these tumors.
