Micetoma of the foot

Introduction: Mycetoma is a nosological entity of infectious origin, chronic, deforming and located more frequently at the level of the lower limbs. The etiologic agents may be true fungi (eumycetes) or filamentous bacteria (actinomycetes). Description: A 52-year-old male patient is presented, with a history of health, with a significant increase in volume of the dorsum of the left foot, who presents multiple small fistulas that drain little transparent liquid and dark solid material such as grains of sand. Serial plain X-rays of the left foot were performed for 14 years, a biopsy of the tumor, a microbiological study, and antifungal treatment was imposed when the etiology was confirmed by Exophiala jeanselmei. Conclusions: Mycetoma is a disease entity with a chronic course, prolonged and expensive treatments. In the present case, without common risk factors such as agricultural work without the proper use of means of protection. Actinomycete infections have been related to conditions that determine immunocompetence deficit.
