Introduction: Cervical lymphatic malformations are congenital anomalies that may present perinatal complications, posing a therapeutic challenge, particularly in cases of lesion rupture. Case presentation: A full-term newborn (38 weeks) presented with a left cervical lesion of approximately 5 cm, with rupture and serous drainage at birth. A staged treatment approach was performed, including necrosectomy and defect closure to achieve cyst sealing, followed by bleomycin sclerotherapy. The patient showed favorable clinical evolution, with progressive reduction of the lesion and complete resolution after a single intervention, without complications. Conclusions: Neonatal rupture represents an uncommon presentation requiring individualized management. A staged approach allows safe treatment, and bleomycin is an effective therapeutic option.
